CONTRACT & HOURS FORM
The YES Contract is updated periodically. By signing the contract, the student is agreeing to stay up to date with the most current guidelines.
Please read the guidelines CAREFULLY.
You will have six months from the date of service to turn in your forms. Forms turned in after six months will be rejected and hours will be unacceptable.
Hours are checked randomly by calling the volunteer coordinator who has signed your YES form. If you are unsure whether an organization's volunteer hours are accepted after reading your contract, please contact the YES coordinator for your year of graduation.
*Under no circumstances will hours completed at home and only verifiable by your parent be accepted for YES credit.
